Another consideration is that the established leagues have an infrastructure in place: fixtures secretaries, competition rules and so on. Setting up a Midland League would be possible, but not simple.
Also, there is the matter that there is only a limited amount of commercial sponsorship available and the jam would then need to be spread between 4 regional leagues instead of 3.
I like the principle of a Midland League, but I suspect that the obstacles could be significant

There used to be a Midland league which included mainly Derbyshire and Notts. clubs; but this was done
away with when Pyramid system was restructured.
